In this new environment, there are new caregivers, a new home environment and residents can experience a feeling of loss of their way of life. Our skilled mental health providers are available to evaluate, diagnose, and treat mental health conditions and provide medication management. They are frequently on site at the facilities and work closely with residents, families and other providers to smooth the transition into the SNF setting and provide compassion and empathy along with skilled mental health care.
