Our residents receive the care of a Nurse Practitioner who works where they live to make sure all they receive preventative care and continued education to reduce unnecessary hospital visits. At Solutions Medical Providers, we want to improve resident care by providing treatment for chronic conditions as well as monitoring episodic and acute events as they occur. We will work with the residents PCP, specialty providers, home healthcare providers, pharmacists, nursing staff and therapy teams to make sure the resident receives seamless care between the disciplines.  Our providers are on site to participate in care planning and interdisciplinary meetings and provide consistent medical follow up to improves resident outcomes.
